Our Services are intended for educational, informational, and wellness purposes and are not a substitute for emergency medical care. Individual results vary based on medical history, adherence to recommendations, genetics, lifestyle, and other factors.

3. Telehealth Consent

By using telehealth services, you acknowledge and agree that: Telehealth involves communication through secure video, telephone, or electronic messaging with a licensed healthcare provider. Telehealth is not appropriate for medical emergencies.  If you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately. Clinical recommendations are based upon the information you provide. Incomplete or inaccurate information may affect diagnosis or treatment recommendations. Telehealth services are provided in accordance with applicable federal and state laws, including HIPAA. You consent to the electronic transmission of personal health information and
acknowledge the potential risks of electronic communications, including delays,
interruptions, or unauthorized access.

4. Mobile and Remote Services

Mobile visits or services provided outside of a traditional clinical setting are offered at the discretion of Ascendance Collaborative. You acknowledge that mobile services may involve inherent risks, including environmental conditions, equipment limitations, and delayed access to emergency medical care. You agree to follow all provider instructions during mobile or remote appointments.

5. Supplement and Product Guidance

Recommendations regarding supplements, nutraceuticals, vitamins, or wellness products are based upon the health information you provide and professional clinical judgment. Unless specifically prescribed, supplements are not intended to diagnose, treat, cure, or prevent disease.

You are responsible for:
 Reviewing ingredients
 Informing your provider of allergies or sensitivities
 Discussing possible medication interactions
 Using products according to provider instructions

Supplement effectiveness varies among individuals, and Ascendance Collaborative makes no guarantees regarding outcomes.

6. Functional Medicine Services

Functional medicine services may include comprehensive health evaluations, laboratory interpretation, nutritional counseling, lifestyle recommendations, chronic disease management support, hormone optimization consultations, and personalized wellness planning. Recommendations are individualized and outcomes cannot be guaranteed. You agree to follow provider instructions and understand that improvements depend upon many individual factors.

7. User Responsibilities

You agree to:
 Provide accurate, complete, and current information.
 Comply with applicable laws regarding healthcare and telehealth services.
 Monitor your own health and seek emergency care when appropriate.

 Maintain the confidentiality of your account credentials.
 Use the Services only for lawful purposes.

8. Payment and Billing

Payment is due at the time services are rendered unless otherwise arranged.
Fees will be disclosed before services are provided.
Unless otherwise stated in writing:
 Fees are non-refundable.
 Missed appointment and cancellation policies may apply.
 We reserve the right to change fees with advance notice.

9. Privacy and HIPAA Compliance

Ascendance Collaborative complies with the Health Insurance Portability and Accountability Act (HIPAA) and other applicable privacy laws.
Our Privacy Policy explains how we collect, use, store, and protect your personal and health information. By using our Services, you consent to electronic communications and secure storage and transmission of information necessary for your care.

10. Intellectual Property

All website content, graphics, logos, text, educational materials, photographs, videos, software, and other content are the property of Ascendance Collaborative or its licensors. No content may be copied, reproduced, distributed, modified, or used without prior written permission.
